[LV EV 5] Operative techniques and tips to prevent and manage potentially life threatening incidents during laparoscopic liver resection

Because most of life-threatening bleeding are bleeding from the hepatic vein, it is essential to divide liver parenchyma avoiding injuring hepatic veins. Thus, we read the grain of branching of the hepatic veins and divide the liver parenchyma by moving the CUSA tip from the root side to the periphery of the hepatic vein. By this manner, at least splitting a confluence between the hepatic vein branches can be avoided. Maintaining the central venous pressure (CVP) lower by reducing infusion volume makes operative field dry and assists safe surgery, then, provides good view for recovering if the hepatic vein is inured. If the CVP maintained lower, a small hall in the hepatic vein trunk can be spontaneously covered by just compressing it for several minutes, while a bigger hall can be lidded by putting a fibrin sealant patch.
