Laparoscopic surgery provides a significant benefit such as much smaller scars, less postoperative discomfort, and quicker recovery times, compared to standard open surgery. Therefore, laparoscopic surgery has already been applied in various surgical fields. In hepatobiliary and pancreatic(HBP) surgery, the proportion of laparoscopic surgery is gradually increasing. Since the first laparoscopic cholecystectomy started in 1985, laparoscopic cholecystectomy has been widely accepted and gradually changed to standard practice . In addition, surgeries such as minor hepatectomy or distal pancreatectomy are performed laparoscopically in many centers. Nonetheless, owing to the complexities of the procedures and a long operative time, major operation such as major hepatectomy or pancreatoduodenectomy has not been widely used in field of HBP surgery. Currently, advances in laparoscopic instruments and techniques encouraged surgeons to perform this challenging procedure as one of the treatment options for HBP diseases. However, laparoscopic major HBP surgery still remain challenging procedures requiring wealth of experience. In particular, there are many difficulties in attempting these procedures in community hospital with insufficient experiences and cases. We introduce our little experience of advanced laparoscopic HBP surgery in community hospital. There were various kinds of advanced surgery performed laparoscopically in our hospital such as left hepatectomy, extended cholecystectomy, excision of the choledochal cyst, conventioanl distal pancreatectomy, radical antegrade modular pancreatosplenectomy(RAMPS), pancreatoduodenectomy and total pancreatectomy. In conclusion, advanced laparoscopic HBP surgery remains a technically demanding procedure, but is also reliable procedure if it is performed with an accurate anatomic knowledge, laparoscopic dissection with meticulous care, appropriate case selection and abundant preparations of laparoscopic HBP surgery.
