[BP SY 1-1] Role of arterial resection revisited

Role of arterial resection revisited: Many patients with pancreatic cancer are diagnosed with locally advanced tumors with arterial infiltration and were previously no candidates for surgical resection. With advances in both surgical resection techniques and perioperative management pancreatic cancer surgery has become safer and more radical. At the same time systemic therapy with modern combination regimens for chemotherapy have become more effective with increased response rates and survival times. Together these advances have allowed pancreatic surgeons to extend indications towards previously unresectable locally advanced pancreatic cancers with arterial involvement. Arterial resections are technically demanding and associated with an increased risk of major complications and mortality. Historically, unacceptably high mortality rates up to 40% were reported. The presentation will summarize recent series on arterial resections from high volume centers and the role of arterial resection and of the alternative of arterial divestment after neoadjuvant therapy. The presentation will address both technical aspects and surgical outcomes of arterial surgery after neoadjuvant therapy for locally advanced pancreatic cancer.
